Only change while the vehicle is not moving. It will not do any harm to try to, to get an idea of the difference. Neutral disconnects the transmission from the wheels. In some vehicles, an auxiliary power take of facility can be installed to eg. run a generator or provide power to external equipment (like a tractor power take off shaft). That can be used with the selector in neutral.

USB sockets work at 5V, it makes no difference if it's a wall power unit with USB out or a computer etc., a phone charge cable will only ever see 5V. How fast it charges depends on how much current the USB socket can provide and how good the cable is. Some USB3 power units can give higher voltages, but only when connected to a USB3 device and when that device requests a high voltage, like for a laptop than needs 16V or 20V. Full USB3 cables have a microcontroller in the connectors, so what they are connected to can tell what the cable itself it rated for ! Any adapter to USB2 or USB3 etc. will only ever give 5V out so cannot do harm.

Most pacemakers monitor the heart rate and do not do anything as long as it stays in a reasonable range. They start triggering heartbeats if the heat slows down more than it normally should. As the it's connected directly to the muscle, it only needs minuscule "shocks" to start each heartbeat, so it's not painful or causing massive muscle jerks like a defibrillator that has to pass power through the whole torso in the hope of getting enough in to the heart muscle. (Think of the electric muscle exercise machines you can buy, that make muscles contract by pads on the skin - they are not directly connected to muscles but still use a fraction the power of a defib as the pads are much closer to the actual muscle. They are not painful or violent; implanted electrodes mean even less sensation). I believe there are other types of pacemaker to control such as attacks of fibrillation, which may deliver somewhat stronger shocks to try and "reset" the heart rhythm.
