Yes, a PhD degree is a DOCTORATE DEGREE.

My husband has a DOCTORATE DEGREE in Pharmacology. He could be addressed as "Dr," although he chooses not to use the title.

People will call you doctor, but you have to decline the sick people.

What else? The Ph. D. the original doctorate. However, in the USA, most of us don't parade our degrees and fellowships unless there is a reason for so doing. That's more for places like the UK and India.

You would, but if you refer to yourself as a doctor with a PhD, you would be a douche.

No one cares. Not once in my 4 years of engineering degree have I heard anyone in science, math, engineering, computer science classes refer themselves as "doctor", nor has anyone in my electives. They typically prefer you refer to them by their first name because they don't want to seem pretentious
