Why would someone refuse to see the doctor if they have cancer?

It depends on what type of cancer it is, and how advanced it is when people make decisions like this. If the person's cancer is of a kind that is usually incurable and the cost is unaffordable, then it's understandable that the person would choose to not undergo painful treatments that would ultimately fail anyway.

Do they actually have cancer diagnosis or just suspect they have cancer.
If they are not yet diagnosed it is either fear or a love of drama.
Fear of cost, death, knowledge ...
Drama of being a victim.
If they actually have a diagnosis and recommended treatment plan then they have made the decision that the costs of treatment are not worth the benefits.
Costs can be financial, emotional and physical.

its their choice to decide how to go in which direction ,,to prolong the outcome, or accept it an live what life is left to the fullest

Probably because they don't want to go through treatment, and let nature take its course. We all make choices.

They may feel that the treatment is pointless and that they are going to die regardless.

If their cancer is at an advanced stage with little hope of anything but prolonging suffering, it may be better to just live as you are instead of sick with chemo. It also depends on the persons age.
