Essentially it just gets mixed with water.
But urine isn’t just urea and water. It also contains excess salts the body doesn't need, cellular debris from the linings of the bladder, urethras and ureter, and a few other components.
If you meant to ask how does the body produce urea, then that’s done by the liver breaking down waste proteins into ammonia and then converting ammonia into urea which is far less toxic and can be discharged by the liver into the bloodstream for removal by the kidneys.
