    Couple of reasons,

Italy and Spain have a lot more elderly. I think the median age was around 48 years in Italy, and 38 in the US. And we know older people get a lot sicker.

These elderly are also living very close to their family. It's not unusual to live with your grandparents, or have them live next door or something. And visiting extended family is a lot more normal.

These countries are also a lot closer, and they kiss or hug instead of shaking hands. So the chance of spreading the virus is a lot bigger, and there is even a theory going around that there are degrees in which you can catch the virus. One droplet causing lesser symptoms than two droplets to put it simply. Them hugging, kissing etc will thus spread the virus more than shaking hands, and cause more severe reactions.

Both of these caused a lot more people ending up in the hospital, causing it to be overwhelmed much quicker, and making it harder to treat others.

They're both hot countries so there's more of a nightlife out there, especially for the old people. Old people in the UK tend to stay in doors as it's always raining or cold or both. Also there are a lot more smokers in those countries and a much larger population of old people.

Lol, some truth in it, but it's not 'always' raining in the UK, not even the wettest country in Europe.

Catholics.  Doing Mass the old-fashioned way means drinking sacramental wine out of the same cup.  It's a recipe for disaster.

Their "Mary" worship isn't helping, either.  "Thou shalt not make any graven images" (2nd Commandment).

Italy was hard hit because they had lots of Chinese tourists.  Both countries are very communal, people spend lots of time hanging out and going to restuaunts (little to no social distancing).  The virus could spread very easily because of this.  Both countries have large old populations who are more likely to die from exposure.

The rates that their health care was overwhelmed played a huge role. They got sicker faster then we did, less time to prepare, figure things out like equipment and supplies and such. As our hospitals become overwhelmed, which is expected, care will have start rationing care, death % will increase.

Did not close borders with China who is a Trading partner, their own healthcare systems that were overwhelmed with even day to day normal healthcare.

I would suspect it is down to the fact that they have a lot more elderly people as part of their population who are more likely to be affected. In other countries with lower life expectancies the impact will be less.
