Why do some people get away with attention seeking social media posts & get attention and sympathy, while others are ignored?

Or labelled attentiion seeking,

Needy, boring etc..

Example someone (not me!!) posting an attention seeking post about having dental surgery..

Choosing to give attention to social media posts or ignoring social media posts all come down to personal choice.  There is no one magical formula people use to deliberately seek out others.  Some people like to be provocative, some like to share all the finer details of their lives and some people like to keep it casual and straight forward.  This is going to attract some and not others.  For all the people that might be interested in someone else's dental surgery, there would equally be as many who would not be interested.

Because those who gain attention and sympathy are well liked and are known to be more charming both in the variety of their posts and in person too.  Attention seekers tend to only post self serving posts and their desperation comes out in the process.  They also tend to be more sensitive to criticism and focus on the negatives instead of taking it all with a grain of salt (not living by the acceptance or denial of social media).
