What ID? any legal documents you need your legal name.
For a driver’s license or state ID card out your full name. So your middle name is on your ID.
My daughter’s friend changed to her middle name when she went off to college. But all legal documents had to be her legal name.

No. You write it as it is on your birth certificate. Same goes for your passport. Tell people what you want. My mom always went by her middle name.

Sure. People do it all the time.
Just don't put in a middle initial.
Or if you preferred, you could list your first initial and your middle name.
Like C. Thomas Howell. Or T. Boone Pickens.

As long as it's not some goverment official document and you must not provide your full name, yes.
