The sentence below reads like she didn't have the ability to become a doctor.

    - She used to think she couldn't become a doctor.

    - She used to think she could not become a doctor.

This sentence reads like there was no possibility of her becoming a doctor.

    - She used to think she couldn't have become a doctor.

    - She used to think she could not have become a doctor.

    Hey San!

IF you spell out the contraction and say both sentences, they are both fine.  The "could not have become" version would be very formal.

Most people would just use the first, especially if using the contraction because it sounds more normal.  Certainly every day speech.

    Hope this helps.
